Life-saving Treatments for Heart Failure Patients
Patients who are diagnosed with heart failure have a support system to assist them in their recovery. Our goal is to keep heart failure patients from being readmitted to the hospital.
Our Heart Failure Telemanagement Program is designed to provide heart failure patients with the education and encouragement they need to follow the treatment program prescribed by their physicians, as well as answers to patients' questions regarding their condition, medications and treatment. More intensive treatment, such as infusion therapy is also available when needed.
Since its inception in 2000, Porter's Congestive Heart Failure Program has helped over 2,500 patients to maximize their quality of life.
PERFECT SCORE ON CORE QUALITY MEASURES
Porter consistently earns perfect scores on the national Core Quality Measures for treatment of patients with congestive heart failure. What that means is that Porter employed the best medical practices for treating patients who have congestive heart failure 100 percent of the time.
